How do I report a work-related injury?

If you have a life-threatening injury, call 911 or go to the nearest hospital

  • Report the injury to your supervisor
  • Call the Early Intervention Nurse at 1-877-742-3467

How do I make an appointment for a strength test?

When a school district has offered you a position that requires a strength test, they will provide a unique link to access the appointment system. 

How do I request a Certificate of Insurance?

You can request a Certificate of Insurance using our Request Form.

 

How do I request a training for my staff?

SIA offers a variety of in-person and virtual training options.  Please review our training topics here and complete the Create-an-Inservice form.

How do I get my Delta Dental Member ID card?

Delta Dental offers printable ID cards on their website, through the Delta Dental mobile app or by mail.  In most cases, you do not need an ID card to visit the dentist.  Connect with Delta Dental to find out more.

Do I need to select a primary care dentist?

Use our Find a Dentist tool to find a Delta Dental PPO dentist near you.

How do I use my VSP benefits?

  • Create an account or log-in to your VSP account to check your coverage and eligibility for your exam, glasses and contacts.
  • Once logged in, click on Benefits tab, then click on My Benefits in the Benefits section.  All your benefit information is listed in that section – what you’re covered for and when.
  • Tell your VSP network eye doctor that you have VSP and they can access your benefit information for you.  You will need to provide your Member ID Number (which could also be your social security number)

Where do I find my VSP Copay/Deductible amount?

If your VSP plan has copays, you will pay them during your office visit.  You may view this information on the View My Benefits section of vsp.com.
